Call-off Request Form
If you are not signed up for work, have removed your name from the list, or are not on the standby list on a day you have agreed to work on, you will receive a point. For every 5 points you accumulate it will equal a write-up.
All call offs must be received more than 24 hours before your scheduled start time. If you fail to do so you will receive a write-up. If you fail to call off and do not show up for your scheduled shift you will also receive a write-up. Three (3) consecutive no shows will result in immediate termination. If you are unable to call or email due to a medical reason you must have someone contact a manager and/or provide a doctor’s note.
